Shipping international samples to Ambry Genetics is a simple process. You may use an Ambry Specimen submission kit or your own packaging and collection tubes if an Ambry kit is not available.
If you are a health care provider or organization, an individual who will have tests conducted by Ambry, or a parent of a minor child or the legal guardian of an individual who will have a test conducted by Ambry, you may request a specimen submission kit. You may request kits by using our website, by calling us at 1-949-900-5798 or by emailing us at KitRequest@ambrygen.com.
Ambry reserves the right to limit and not to accept or respond to any request for a submission kit for any or no reason, including if the request does not clearly identify the individual or organization requesting the kit. By submitting a request for any submission kit, you hereby agree to these terms and also the Privacy, Legal Notices and Disclaimers found at: ambrygen.com/legal.
Complimentary Kits: Ambry may not charge you for receiving submission kits; and Ambry may not pay for sending international shipments to our laboratory. All country customs rules will be reviewed on a case by case basis.
Shipping: In general, requests for kits that we receive before 4:00 PM PT will be shipped the same or next business day. Submission kits are shipped internationally via FedEx and arrival will be contingent on international shipping conditions. If you require kits in an expedited manner please indicate this in the “Special Requests” field below and we will collaborate with you on the most efficient process, but there is no assurance that you will receive the kits in a guaranteed timeframe.
You may also use and ship to Ambry your own packaging and collection tubes if an Ambry kit is not available, provided your packaging and collection tubes meets our requirements. Refer to specimen requirements form for more details.
Expired: Please do not use any contents of the kits beyond their printed expiration date. Please refer to label on outside of kit box for details.
Specimen Kit Components: Kits are either Blood or Saliva kits (saliva kits contain 1 or 2 tubes). Most tests require either 1 blood tube or 1 saliva container. All blood related submission kits contain 2 blood collection tubes and the ability to ship up to 5 tubes per kit (as of 8/1/2014); the second blood/saliva tube is needed for Exome family testing, Multi-gene panels (Ex: “Next” tests), and redraws. Refer to Ambry’s specimen requirements form for more details and minimum required volumes.
Saliva Kits: Submission of saliva samples can possibly extend the turn-around-time of the results by an average of 2 business days.
Online Ordering: Find the submission kit type(s) you would like to receive and specify a quantity for blood and/or saliva kits. If you do not find a kit quantity that you are requesting, please make a note in the Special Request section below stating how many kits you would like, whether blood or saliva, how many tubes per kit (blood kits can hold up to 5 tubes, and what style submission kit you would like (i.e. 1 kit, blood 2 tubes, General Submission Kit). For organizations requesting kits, if you would like an auto-replenishment of kits, please give us the minimum, maximum and the current inventory your organization has in stock. Our team will aim to auto-replenish your kits when the minimum threshold is met.
Authorized Health Care Professional’s Order: Ambry will not process any specimen for testing without a signed requisition form for the requested test from a physician or other health care professional who is authorized to order the requested test and verify that the patient has received informed consent. Ambry will not conduct the test and reserves the right to discard the specimen if there is no such signed requisition form. Authorized Patient Consent: Ambry will not process any specimen for testing without the signed, written consent of the individual whose specimen is to be tested, this consent may be provided by the physician or other healthcare professional attesting to the presence of a signed informed consent. For insurance billing purposes, patient consent to bill insurance is required on the requisition. Ambry will not conduct the test and reserves the right to discard the specimen if there is no such signed consent. Submit: For online kit requests, please complete all required fields including name, shipping address, email and phone number. Once completed, you will see a message confirming your submittal was received.
