Ordering Process Steps: Order kit, ship sample to Ambry, lab completes testing, results available!

  • Login to AmbryPort or create an account*
  • Complete the appropriate fields and submit your order
  • Ship the Sample Submission Kit using the pre-paid FedEx or DHL** envelope

  • Download a copy of the test requisition form or use the form provided in your Sample Submission Kit
  • Complete the fields to ensure no delays in processing
  • Ship the Sample Submission Kit, with the completed TRF, using the pre-paid FedEx or DHL* envelope. If using insurance billing, please provide clinical documentation and copies of the insurance card

Ambry understands each patient situation is different and that collecting a specimen in-person may be a challenge. We provide several different options for your patients to easily submit their specimen for genetic testing.

1. Have a saliva kit shipped directly to your patient

Most Ambry genetic tests** can be completed with a saliva sample kit shipped directly to your patient.

  • Order in AmbryPort and have a saliva kit shipped directly to your patient. When placing an order in AmbryPort, in the Specimen section, add a Saliva specimen type and select ‘Yes’ the patient needs a kit sent to them.
  • We will ship a saliva kit directly to the patient via FedEx and include a copy of the patient signature card, pre-paid return shipping label, and return envelopes for USPS and FedEx. You will be notified via Ambry Message as soon as the saliva kit has been shipped to the patient.
  • Once we have received the patient’s sample, turnaround times, billing verification, and report delivery will proceed as usual.
  • If you prefer to send us a hard copy test requisition form, please note ‘Ship Saliva Kit to Patient’ on the form and send it to us via fax +1 949-900-5501 or email info@ambrygen.com.

2. Patient can visit a local draw site

  • Order the testing in AmbryPort and reach out to Customer Support to inquire about available local laboratories that could collect your patients sample. Ambry partners with AnyLabTestNow, and most of their locations are willing to collect samples for our patients.
  • If there is a location nearby, the Ambry team will ship the patient test kit to their home address.
  • The patient should call ahead and make an appointment with the AnyLabTestNow center as walk-in appointments are limited due to social distancing precautions. Ambry staff can assist with providing the location details, including address, operating times and phone numbers.
  • On the day of their blood draw, the patient will need to bring in their test collection kit as well as a completed test requisition form. If you need Ambry to send a copy of the completed test requisition from via email for the patient to print, please advise the customer service team.
  • The local lab draw facility will collect the patients sample and send it to Ambry.
  • Once we have received the patient’s sample, turnaround times, billing verification, and report delivery will proceed as usual.

3. Request a mobile phlebotomy service go to your patient

  • Order in AmbryPort and request mobile phlebotomy services be provided.
  • When placing an order in AmbryPort, in the Specimen section, add a blood specimen type and select ‘Yes’ the patient needs mobile phlebotomy.
  • We will ship a blood collection kit directly to the patient via FedEx and include a patient signature card, pre-paid return shipping label, and return envelope.
  • Ambry will coordinate mobile phlebotomy services with the patient at no cost to them.
  • Once we have received the patient’s sample, turnaround times, billing verification, and report delivery will proceed as usual.
  • If you prefer to send us a hard copy test requisition form, please note ‘Request Mobile Phlebotomy’ on the form and send it to us via fax +1 949-900-5501 or email info@ambrygen.com.
